Settings in d20 Modern:
Shadow Chasers was originally a Pollyhedron Magazine mini-game.
Agents of Psi is a new setting to the best of my knowledge.
Urban Arcana is another new setting, though it is basicall D&D set in the modern day. (Though it's origins may be tied to the Amazing Engine setting called "Magitech")
Settings in the d20 Menace Manual
Dark Matter is not presented as a setting in the Menace Manual, but many creatures and organizations from this old Alternity setting appear in the Manace Manual. It also appeared in Pollyhedron #167. Dark Matter is itself an evolution of the earlier Tabloid setting for TSR's Amazing Engine RPG.

Bug Hunters was originally a setting for TSR's short lived Amazing Engine generic RPG.
Dimension X is a new setting to the best of my knowledge, but may have taken some inspiration from the earlier Tangents book for TSR's Alternity game.
From the Dark Heart of Space is another new setting, though it is highly reminicent of Fading Suns
Gene Tech was originally a Pollyhedron Magazine mini-game found in Polyhedrom #155 (in the back of Dungeon #96)
Mecha Crusade was originally a Pollyhedron Magazine mini-game found in Polyhedrom # 154 (in the back of Dungeon #95)
Star Drive was originally a setting for TSR's Alternity Game
Star Law is a d20 remake of TSR's old Star Frontiers RPG.
The Wasteland is a post-apocalyptic setting that probably owes it's origins to TSR's old Gamma World RPG and the Omega World minigame (also a decendant of Gamma World) that was found in Polyhedrom Magazine #153 (in the back of Dungeon #94).
Settings in d20 Past:
Age of Adventure is based on the golden age of piracy, and not based on any specific previous games that I am aware of.
Shadow Stalkers is a victorian setting, probably influenced by TSR's old "For Farie Queen and Country" victorian setting for the Amazing Engine RPG. It may also have been influenced by Call of Cathulu.
Pulp Heroes was originally a d20 minigame found in Dungeon #90 and Polyhedrom Magazine #161 (found in the back of Dungeon #102).
